As part of a Masters research project, I am exploring the usability of the current Healthlink/National General Referral form in the context of the Radiology and Primary Care setting. The architecture of the existing National General Referral form is no longer fit for purpose as a Radiology referral. Much of the information supplied is unnecessary and irrelevant, and creates a clinical burden carrying out the vetting and justification process. Your opinions and perspectives on the use of this form will contribute greatly to this research.

At present, a General Practitioner uses a National General Referral Form to refer a patient for diagnostic imaging. The referral is sent electronically to the Radiology department via Healthlink. This survey seeks your opinions and perspectives on the usability of this referral.
The data collected in this questionnaire is anonymous. Individuals and institutions will not be readily identifiable. Hospital location is only included to ascertain the geographical spread of respondents.
The data collected will be analysed and reported with the aim of providing insights into the opinions and perspectives of Radiologists.
You are free to exit the questionnaire at any point, however, once the questionnaire has been submitted, you will be unable to withdraw after this point.
